GIFT CARD POLICY
NEW PATIENTS
- To redeem a new patient special offer, the offer must be mentioned when scheduling an appointment.
- For new patient gift cards, the gift card will be sent by email after the appointment is completed.
- Please provide a valid email address on your new patient paperwork to ensure you receive our offers!
- Only one referral source per patient will be accepted.
- Special offers cannot be combined.
- Gift cards will not be sent after the end of the special offer month that the appointment occurred.
- If you did not receive your gift card, please email smile@costasmiles.com before the end of the month that your appointment occurred.
- Gift cards and discounts cannot be applied to appointments retroactively.
REFERRAL PRIZES
- Each patient referral counts as one entry into our monthly drawing.
- Patient referrals can be family or friends!
- All of the referring patients will be entered and one winner will be drawn randomly.
- Winners are announced within the first week of the following month.
- One winner from each office will be selected and notified by email.
- Gift cards will be sent by email to the winners.
