Please read this before reserving an appointment:
1.) If you are a new patient, please click here to complete your paperwork. This must be done before your virtual appointment.
2.) If you are an existing patient, or a new patient and have completed your paperwork, please click here to pre-pay for your appointment.

Enter $110 as the amount to pay. This is a flat fee for all virtual appointments and you will be reimbursed according to your insurance.

3.) AFTER you have completed steps 1 and 2, please select the date and time of your choice.
4.) Once your payment has been accepted, you will receive a link from our office to begin your virtual appointment.
5.) Click the link at the bottom of the email to be directed to our virtual waiting room.
6.) Please “check-in” to our virtual waiting room
Make sure your microphone and webcam are enabled and wait for the doctor to join.
